Forms to Apply to IS3D


In order to join the IS3D major you will need to complete the Custom Plan form. Filling out this form is your application to the IS3D major. Please make sure to have your degree evaluation open while you complete this form so you can reference information such as your current GPA and any courses you've previously taken.

  • This form will ask you which three disciplines will be part of your Custom Plan and what classwork you will use to satisfy these requirements. After you complete the form you will receive an email letting you know your next steps are. If you are missing requirements, we will let you know what coursework to focus on, and if you meet our eligibility requirements we will start moving you into the IS3D major.

Early Application Form

Occasionally, we allow students into the major if they do not meet all of our eligibility requirements, but only in cases of a serious or compelling reason. If you believe you qualify for early admission under these circumstances, please fill out the Early Admission form. The form will ask you to complete a Custom Plan, identify how you will meet the outstanding requirements, and upload a letter of support from your academic advisor.

Forms for Current IS3D Students

If you need to make changes to a Custom Plan that has already been submitted please use this form. You can use this form to make changes such as:

  • Add and remove courses from your Custom Plan
  • Change a discipline
  • Change the unit distribution on your Custom Plan (for some students)

Because IS3d is an individualized major we cannot provide you with a simple roadmap. Please use this spreadsheet to better understand how you can complete your IS3D coursework. On this form you can keep track of which classes are part of your Custom Plan, and plan out how you will complete your coursework semester by semester.
